Samuel Allen Forman, 31, was born on 9/18/1988, and got his “Angel Wings” too soon passing away Aug. 7, 2020 at his home in Huntington Beach California due to complications from COVID-19 he’d already beat.

Samuel was born and raised in Kingfisher, OK and had three older brothers and two sisters.

“Sam” as he was called - graduated from Kingfisher public schools in 2007 then attended the University of Central Oklahoma and Oklahoma State University.

Most recently he obtained his insurance license working full time for a State Farm agency in Huntington Beach California while working at Trader Joes in the same area.

He enjoyed his “Trader Joe’s family” and he so valued spending time with them plus his extended family, including his roommates Wren and Josh he loved like brothers.

Sam had an abundant faith in his Lord Jesus Christ and freely shared how his faith guided his life through all his ups and downs knowing God was always by his side.
